Patient Centered Medical Home Recognition

Angel Kids Pediatrics of Jacksonville, FL, has been awarded recognition by the National Committee for Quality Assurance Patient-Centered Medical Home Program.

“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane. “Recognition shows that Angel Kids Pediatrics has the tools, systems and resources to provide its patients with the right care, at the right time.”

What is a Medical Home?

A patient-centered medical home is a model of care that strengthens the physician-patient relationship by replacing episodic care with coordinated care and a long-term healing relationship. Each patient has an ongoing relationship with a personal physician who leads a team at a single location that takes collective responsibility for patient care, providing for the patient’s health care needs and arranging for appropriate care with other qualified clinicians. The medical home is intended to result in more personalized, coordinated, effective and efficient care. A medical home achieves these goals through a high level of accessibility, providing excellent communication among patients, physicians and staff and taking full advantage of the latest information technology to prescribe, communicate, track test results, obtain clinical support information and monitor performance.

We have worked hard to provide our patients with more access to care and information. For example:
• Late hours and weekend appointments • Same day sick appointments
• 24 hour access to medical information through Patient Portal • More in-office services such as, bilirubin monitoring, labs, and allergy testing
• Seven convenient locations to serve the Jacksonville community

Don't Just Take Our Word for it!

We are so proud of what we do, not only for our patients in all our offices, but by our mission and philosophy. Our practice has been recognized by the following prestigious organizations.

"I saw the global picture in the need for pediatricians to be involved, not only with the child but also with the child's immediate family and extended family - the community. I recognized that you must do more than treat ill and undernourished children, you must educate the parents and the community to instill in them the idea of preventative care."
